Tunable Diaphragm – the Heart of the Cardiology III Stethoscope
With its innovative tunable diaphragm technology, the Cardiology III™ provides both bell (low frequency) and diaphragm (high frequency) capability during use. Switching between bell and diaphragm modes is as easy as a simple pressure change on the chestpiece, which means there is no need to remove, index and replace for same-site auscultation or turning over the chestpiece. This time-saving feature allows for focus on the patient. The diaphragm on the small side is easily removable and, with the included nonchill bell sleeve, converts to a traditional open bell for capturing low frequencies. The Cardiology III™ stethoscope is used by students and medical professionals such as cardiologists, pediatricians, and other healthcare providers to identify, listen to, and study cardiac, lung, and other body sounds in adult, pediatric patients. The diaphragm on the small side is easily removable and, with the included nonchill bell sleeve, converts to a traditional open bell for capturing low frequencies. What is a Stethoscope?
Stethoscopes are diagnostic instruments that amplify sounds made by the body from the heart, lungs, abdomen and intestinal tract, or other body sites. A stethoscope is used to detect and study internal systems, such as fetal development, blood flow or blood pressure (BP), and respiration, and sounds, such as cardiac, venous, arterial, pulmonary, and uterine. Stethoscopes typically have Y-shaped rubber tubing that allows sound to travel from the chestpiece to the eartips. An open bell detects low frequency sounds, and a diaphragm detects high frequency sounds. A stethoscope can come as single, double (dual), or triple-head. Binaural stethoscopes are used with both ears, and single stethoscopes work with one ear. Differential stethoscopes are double-head, and allow sound comparison between different body sites. Non-electronic, also called mechanical or acoustic, stethoscopes transmit sound through hollow tubing to a listener’s ear. Stethoscopes are used by students and a wide array of healthcare providers including physicians, nurses, and veterinarians, as well as specialists such as cardiologists, pediatricians, gastroenterologists, pulmonologists, and respiratory professionals in hospital, clinical, or other healthcare facilities.

Littmann cardiology III stethoscope. This outstanding, versatile stethoscope features two patented tunable diaphragms (adult and pediatric) for listening to both low and high frequency sounds. Designed for acoustical excellence, the "two-tubes-in-one design" eliminates noise artifact that can be created by the tubes rubbing together. The pediatric side of the chest piece easily converts to a traditional bell by simply replacing the diaphragm with the nonchill bell sleeve included with each stethoscope. I've had mine for about 3-4 years. You can pick up murmurs and crackles very easily with this model. Recently, I tried using my former stethoscope on patients (Sprague Rappaport), and WHOA!! I could barely hear anything!! (I had gotten spoiled by my Cards-III for the last few years without even knowing it!) Years ago, I was wondering how my classmates and residents (all of whom seemed to have the Cards-III) could hear murmurs and stuff, and I couldn't! As a resident now, I'm glad I own one. One day, I forgot my own Cards-III, and had to borrow a colleague's stethoscope (it was a Classic II SE). Even though her Classic II SE was also a Littman, I still couldn't hear as clearly through that, as I did the Cards-III.

On the downside, it is rather pricey. But I equate it to buying a good pair of glasses. In exchange for your money, you'll be able to hear a lot more.

I loved the versatility of being able to switch from adult to pediatric just by rotating the stem of the chest piece. The quality of auscultation is good. The only problem is that the tubing got stiff after a couple of years and cracked after 7 years. Apparently a vinyl protector is a must with these stethoscopes.

The Littmann Cardiology I got in med school back in 1988 bit the dust five years ago (tubes cracked after 19 years of heavy use). I got a top-line Tycos that worked for 5 years before it just fell apart (tubes hardened and cracked and the headset spring pooped out). The Tycos acoustics seemd OK but not as good as my first Littman, so I went back to Littmann, and I'm glad I did. The Cardiology III transmits heart, lung, and abdominal sounds far better than the Tycos. I thought my older ears were the reason the Tycos didn't seem as good, but no I know it was the scope, because I hear sounds just as well as I did years ago with the Cardiology... The tunable diaphragm takes getting used to, but it does work as advertized. I'm a neurologist and I find that the "pediatric" headpiece with the smaller tunable diaphragm is superb for listening to carotid and femoral bruits, and even for listening to flow in the temporal arteries...You can remove the small diaphragm and use that end as a bell, but frankly I don't see a need to do that.

Bottom line: you can't find a better acoustic multiuse stethoscope. I would have been satisfied even if I paid the list price. You can find it cheaper on-line. I purchased mine in a brick-and-mortar store in Oakland, CA (Nightingale Scrubs) for $100 below list, and I'd recommend them to local folks who want to go there. They carry the full Littmann line.

I just wanted to say I bought this stethoscope 10 years ago and it works very well. I'm a little hard of hearing so I bought this one and I'm glad I did. Whenever I borrowed another nurse's stethoscope, I appreciated the superior sound in mine comparatively. The fortunate thing is that most of the parts like the diaphragms, earpieces, and traditional bell collar are replaceable and relatively inexpensive. The only advice I give is this: NEVER STORE YOUR STETHOSCOPE IN A COLD CAR AND ALWAYS KEEP THE TUBING COVERED OR OFF OF YOUR BARE NECK. The extreme cold can and will crack the tubing. I wouldn't leave it in a hot car either for that matter. Also, bare skin on your neck making contact with the tubing over time will damage the tubing and cause it to become very hard, stiff, and crack. It even says this under "Care and Maintenance" on the Littman website. So, always cover your stethoscope with a fabric cozy or wear a collared shirt to provide a barrier. I wish I would have known this earlier or that this important piece of info was in the user's manual. On my tubing, you can clearly feel the difference between the soft parts of the tubing that didn't rest on my neck and the part that did which unfortunately became stiff and cracked. You can replace the tubing, but it is expensive compared to the other replacement parts. Learn from my experience and take care of your stethoscope the right way and it will last for many years.

This steth is amazing when it comes to acoustics. Everything you want to listen to is crisp and clear. I often have my fellow nurses borrowing my stethoscope when they are listening for clicks and murmurs. I honestly don't know that you could find a better stethoscope that isn't electronic.

I find the soft ear pieces to be comfortable, but it did take a bit of stretching it out in the beginning to keep it from squeezing my head too much. Once it stretched out a bit, the tension has been great and applies just enough pressure to stay in my ears and create a good seal without me feeling like it is burrowing into my head.

Personally, I don't really use the tunable diaphragm. When I first got the stethoscope I attempted to use it and I never got the hang of it. I have an open bell on the smaller side and it works just fine for me.

The downside is that this is an incredibly heavy stethoscope. I can't wear this thing around my neck at all because my neck will start to ache in a short time. I feel like the constant pulling it in and out of my pockets coupled with constantly putting it down every where greatly increases the likelihood of it disappearing and means it is getting far dirtier than if I could just throw it around my neck.

Another feature of this stethoscope that i don't particularly care for is the stickiness of the tubing. I clean it several times a shift, but it always seems to have a tackiness that I don't care for. It is so tacky that it actually picks up lint and stray hair. I contacted Littmann about this and they stated that the material that the stethoscope is somewhat tacky due to the nature of the material.

This is a great stethoscope that has served me well, but am shopping for the Littmann Classic II S.E. as a replacement.
