I installed this for its intended purpose in our new structured wiring closet. Phone line goes into one jack, and the other 12 connect to the wiring in the house.

Only issue is mounting it. One can purchase a bracket from OnQ/Legrand that this clips into, but that doubles the cost. And it uses these proprietary plastic "rivets" that appear to connect it easily but mean that it's hard toconnect to anything else. The included mounted bracket holds only one end, not both, so it hangs a little loose.
