Red Vertigo is the fifth film in the filmic versions of Ishii Takashi's manga series Angel Guts, and unlike the previous four films which were directed by Sone Chusei ( I and II), Tanaka Noboru (III), and Ikeda Toshiharu (IV), the fifth, and later the sixth film, was directed by Ishii himself whose name is best known for directing Gonin starring Kitano Takeshi. Angel Guts: Red Vertigo again focuses on the life and sexual misadventures of Tsuchiya Nami who, in this incarnation of the series, is a young nurse working the nightshift. Exhausted day after day because she helps her photographer boyfriend develop photos of nude models, Nami is targeted by a couple of patients as a prime rape victim. Even though they manage to rip off a portion of her clothes, Nami is able to escape, however, when she returns home, she not only discovers her boyfriend sleeping with one of his models, but she also overhears him state that he has no real relationship with her. A distraught Nami runs away and unfortunately for her, she is struck by a car...

The car is driven by a fellow named Muraki who is in deep trouble after embezzling millions of yen from his company. With the company management looking for him with the threat of bodily harm, Muraki flees and accidentally strikes Nami with his car. Instead of leaving her beside the road, Muraki hoists her into his car and drives away. Certain that she will remain unconscious, Muraki binds Nami's hands and attempts to rape her while she is out cold. Fortunately for her, she awakens and is able to get Muraki off of her. Unfortunately for her, however, Muraki takes her to an abandoned building and again tries to rape her. However, although Muraki has tried to rape her twice, Nami falls for the man and the two decide to run off together. Yet, can they truly run away from their troubles?

As with the first four films of the Angel Guts series, Red Vertigo focuses on rape, and like a number of other pink films, the raped woman falls for her attacker. Why does she fall for him? That is hard to say, but the film feeds many aspects of darker male fantasies and displays there warped nature on screen.
