Standard Oil started this series of process safety booklets in the early sixties to pass out to employees, for the purpose of communicating lessons learned from catastrophic and sometimes tragic accidents that occurred inside their fenceline. British Petroleum (BP) kept it up and decided to make them publically available. In the oil and gas industry, many things are legal and some things are sacred. Most companies would consider this type of information "legal" - company confidential *only* information to reduce the chance of their positive and important message from being twisted in some way as the basis for a frivolous lawsuit. BP is to be commended for considering this information "sacred" - the kind of stuff to be shared with their competitors (like myself) regardless so that we can mitigate a risk instead of learning from the same mistake. This is a very responsible thing to do. It represents a moral standard for others to follow. I constantly refer to these books in safe design, operation, project development, and PHA/HAZOP effectiveness. You will too.
