This book is terrible. The publisher, General-Books, simply scanned the original book. They did not proof their scan, nor did they make even a small ateempt to square it with the index.
As a result, this book is confusing and, in places, diffficult to understand.
I would not recommend that anyone purchase this book. Save your money and borrow the original from a libarary.
