Very helpful and a good place to keep the info. Though I had some of the info gathered, this notebook prompted me to get other info gathered. I take this notebook with me to every Dr. appt. or any time my care receiver goes to the hospital. It REALLY helps ensure he gets the best care.

Since I'm a spreadsheet geek, I put the info into my usual spreadsheet and print it out every time it is updated.
