Donald K. Freedheim, PhD, is Professor of Psychology at Case Western Reserve University. He is also the editor of History of Psychotherapy: A Century of Change.

Jeremy P. Shapiro, PhD, is Vice President of Research, Quality Improvement, and Training at Applewood Centers, Inc. in Cleveland, Ohio.
