This book was a life saver for me as I opened my solo practice this summer. Without it, I would have spent countless hours developing these forms myself. The feature that allows you to customize the forms is especially nice. I recommend this for new or veteran mental health workers.

The software does NOT work on an iMac. When used with Windows Vista, the forms are available to customize, however, they need streamlining and updating with regard to fonts, spacing, etc. The documents don't look as if they have been updated aesthetically since the first edition of the book. Disappointing.

I was hoping that this book would have some quick & easy, down & dirty, managed care friendly forms to use, and that the enclosed disc would let me modify them for my practice. Not so. The "up to date" (please note the sarcasm!) forms entailed loads of writing - and were not anything that would help speed up the paperwork process. The disc? What a joke. If you just want to spend money, take yourself out to eat - don't waste it on this book!

I ordered this book five weeks ago and it was the best thing that I ever did for school. The book is helpful and can be used throughout ones career. There is so much information and every form imaginable you will need for counseling the drug dependent client with example forms as well. Excellent book even if you are not required to have this book, just get it anyway.

I had to find an office and begin seeing a few patients 6 months before I had planned to open my part-time private practice. Having read many books on the topic, I felt ready to get going. Alas, I did not want to spend a lot on a computer office management system when only seeing a few clients for now. Thankfully, after reading the reviews on Amazon.com, I found the Clinical Documentation Sourcebook. Due to fast shipping, I was able to be fully prepared for my first client. The book had easy to use forms that I could duplicate. I was able to utilize many forms on a variety of issues that I hadn't even considered. I am very pleased. I would recommend this book to others as well.

This sourcebook is the best one I have found to assist with beginning a clinical practice and producing legal documentation.

As a clinical psychologist in independent private practice, I have always been interested in anything that can keep me ahead of the documentation curve. I found this book to be very helpful. The forms included on the disk were easy to modify for my specific needs. Why re-invent the wheel?

This is a great resource for therapists at all levels. It is relatively extensive. The documents that I have used have been very thorough and well-designed to be easy to follow.

This book provides an abundance of forms for private practice use. It comes with a cd that includes all forms for reproduction.

I can only use ideas of this book because I would need one which is more tailored towards the Australian mental health system. Nevertheless it is very useful for me to find documentation strategies for my newly set up private practice rooms. I would purchase this book again.
