About the Author

CHRIS E. STOUT, PsyD, is a Senior Vice President of Clinical Applications, Policy, and Development at Forest Health Systems in Des Plaines, Illinois. He edited The Complete Guide to Managed Behavioral Healthcare and is the author of eight other books, including Psychological Assessment in Managed Care.

ARTHUR E. JONGSMA, Jr., PhD, is the founder and Director of Psychological Consultants, a group private practice in Grand Rapids, Michigan. He has more than 20 years' experience with mental health treatment, and is the coauthor of The Complete Psychotherapy Treatment Planner and three other books.
