Used this with Dragon Medical 10.1 and it works with much higher precision that I expected. With Dragon installed, there are no extra drivers needed when installing this, just plug and go. Having navigation buttons make moving around the EMR smoother, and the dedicated record button prevents users from needing to speak to wake up the mic.

Very expensive device and the cord is very long. Good when the computer in on the floor and the physician is standing at a counter, but can become unmanageable when using a laptop on the same counter with 8 ft of spare cord laying around.

I am a physician and use this for transcription right into my clinic's EMR (EPIC). It is over-priced, yes...but the accuracy you get out of this thing is amazing. The quick buttons are nice for access to portions of your note, and can be configured how you would like it. I wish it was half the price, then it would get it 5 stars, but for what it is, it is great.

Description says "Only to be used with Dragon Medical 10". However, for those still have Dragon Medical 9.5 and don't want to upgrade yet, you can still use this Powermic II with DNS 9.5 under Windows 7 and Windows 8 64 bit versions. Here is how:

1. Follow the instructions here to modify the installation file with Orca, complete installation of DNS 9.5:

2. Go to the program folder `C:\Program Files (x86)\Nuance\NaturallySpeaking9\Program' and find the Audio.exe file, change property to win XP sp2 compatible and run as administrator. With this wizard running, click "Prepare User for Dictation", finish the wizard. Dragon will freeze after this, force the program to end with Task Manager.
3. Dragon will prompt you for audio wizard again, cancel and open "Prepare User for Dictation" again, you should be able to complete calibration and initial training.
4. To use Powermic II under Windows 8 with DNS 9.5, download and run this hotfix:
healthcare1content.nuance.com/downloads/PowerMic/Hotfix1.exe
With this file running, go to your temp folder and find power_mic2.cab file in a folder with a weird long name, extract two files: hiddev.dll and usbmgr.dll, copy these two files to your DNS program folder and replace the old ones. Launch DNS again, you'll see Powermic II listed under dictation source, select it for a new user or as a new source for an existing user. A new tab for Powermic will be added in "Tools/Options" dialogue box for basic button configuration.

Notes:
Uncheck "Enable commands in HTML windows" in option will let you dictate directly into Practicefusion in Internet explorer, but performance is poor. Use Chrome for Practicefusion, DNS 9.

Fortunately, I didn't have to pay for my Dictaphone PowerMic. If I had, I probably wouldn't have one, because the price seems astronomical. That said, I find that it works well -- much better than headset mics. What makes it nearly worth the price, though, are the navigation buttons that make it effortless to jump from one section of my medical-note templates to the next. THAT's the big time saver with this microphone, but the dedicated on-off button is also very nice.

This is the second one I have owned. I use it all day and all night. My last one developed a short in the cord which was disappointing. Depsite this the mic is really great which is why I was willing to buy another when my first one broke. The ability to customize the buttons and the accuracy on Dragon is awesome.
