Introduction ______________________________________________________ Pilot Error. Words aviators never want to hear. Our training should make us impervious to mistakes, but there is not one pilot alive who can say he or she never made a mistake. When an error results in an aircraft incident or accident, we question why the pilot ignored information or made a wrong decision. Can factors relating to human physiology contribute to flaws in performance?

I have flown both high altitude aircraft and unpressurized recips. Through my twenty-plus years in aviation, I have discovered many human factors that contribute to the pilot error equation. When we speak of proficiency, most of us hear the words recurrent training. As professional pilots, we take check rides, sit through ground schools and get the sweat rung out of us in simulators. This should make us impervious to pilot error. Why then, are our heads up and locked on occasion? In my early flying days, I once pulled the mixture out instead of the carburetor heat while flying my Stinson over the Verrazano Narrows Bridge in New York.Instantaneously, I realized my error and fixed the problem, but I never forgot that incident. What caused my inattention? I was proficient and extremely familiar with my aircraft. Something must have short circuited my brain at that moment.

Any pilot reading this must have their own dark secrets of pilot error. Fortunately, most of us solve the problem before we become a statistic. A small percentage of pilots don?t survive the errors, or they lose their license because of a BIG mistake. Because I never want to be this kind of statistic, I began a quest to find out why I may be prone to pilot error even if my piloting skills are proficient. Although some of the information presented is targeted at pilots operating in high altitude environments, this book is written to benefit all pilots.

This handbook is for general information. It was designed to be brief, and give essential information regarding physical conditions that can contribute to situations where pilots make mistakes. Research data from a variety of qualified sources has been compiled here for easy access. Included is a suggested reading list and bibliography to pursue if you need definitive information on a specific subject. We are planning to update this book as new data becomes available; therefore, we welcome personal discoveries. -Nina Anderson

This book is a great reference guide. I used it with the video Human Factors & Pilot Error and it helped me to recognize lots of things I do that can affect my concentration when I fly. I recommend it to every pilot, especially flight instructors who want to teach their students right.

Well-publicized accidents only draw attention to how many factors play into our ability to control an airplane. Rarely taken into account is the type of foods eaten or not eaten prior to flight, the chemical exposure of the pilot, the ionization of the atmosphere and even jet lag symptoms. This book is a wonderful guide that all pilots from Cessna 150 flyers to 747 drivers can benefit from. They will get food for thought and information that can keep them at their top physical and mental condition before and during their flight. The book can be used as a text book along with an instructional video...Human Factors and Pilot Error that was filmed at an FAA Safety Seminar in 1999.
