Format: Paperback
Despite the odd title, this book is quite well written. It's not meant to be a reference, but it's not just a high level overview, either. The explanations and the sample code are good enough to give you an idea of what you can do and how to get started. That's assuming you don't conclude that HealthVault is overkill for your simple self-tracking app...

I got the kindle edition, which I thought was very well done. The balance between technical detail, PHR background and simplicity was struck rather well. If you're looking to learn more about HealthVault and PHR use in general, this is a pretty decent start.
