The objective of this book is to restore FMEA as a vital development tool within the organization's quality and reliability programs. This objective will be accomplished by introducing new developments that 1) make FMEA accessible to more people in the organization, 2) improve the efficiency of constructing the FMEA and 3) increase the effectiveness of information extracted from the FMEA to make significant gains in quality and reliability. The first case study demonstrating these new developments was present in a paper (No. # 940884) at the SAE (Society of Automotive Engineers) International Congress in Detroit in 1994. It should be noted that these new developments in FMEA can provide significant benefits to all industries. Examples in the automotive industry, medical industry, food industry, as well as the service sector will be provided throughout this book. Some of these new developments introduced in the book may be in direct conflict with some of the traditional approaches to doing FMEA. I would ask that the reader, when comparing these new developments against the traditional approach, make comparisons from the customer's perspective and, more importantly, do an actual application. I am confident that the reader will realize the benefits associated to these new developments."
