Editorial Reviews

Review

"The information contained (in For the Record) is quite extensive and extremely valuable for anyone who must determine what is important to an individual or a family. All too frequently we find that there are no records of the important items in one's life and a product like For The Record fills the need when it becomes important. Who among us has not had the questions come up about what a sick or deceased loved one would want or demand depending upon a certain situation? For The Record offers a simplified yet complete system of organizing someone's life. Great work." --Darryl J. Roberts, author of "Profits of Death: An Insider Exposes the Death Care Industry

"I work with many older adults who will find For the Record extremely beneficial in helping them organize their lives in such a way that will make it very easy for them to access important documents... (It) has a very enticing format, is self-explanatory, and the forms are quite easy to read and use. I plan to recommend For the Record to a number of my patients, and to anyone who needs to get their life affairs in better order." --Michael Cofield, Ph.D.,Director of Behavior Medicine, SunHealth Corporation, Sun City, AZ

"A great tool for organizing personal business information. In an emergency, it can be easily found and used by a caregiver or someone managing the affairs of an individual... It collects all the important information in one place." Roberto Armijo Executive Director Community Information and Referral Service, Phoenix, AZ --Roberto Armijo, Executive Director, Community Information and Referral Service, Phoenix, AZ

About the Author

Ricki Pagano has devoted her life to helping people cope with problems not of their making, problems that could befall any one of us. Ms. Pagano graduated from the University of Arizona in 1981 with a Bachelor s Degree in Psychology, and three years later received her Master s Degree in Social Work from Arizona State University. She spent the next 15 years working in the field of medical social work where she developed an expertise in, and love for, the area of geriatric care. It was here that she learned how essential it is to our own sense of well-being that we are able to maintain control over our own lives. And it was here that For the Record was conceived as a valuable tool for helping people regain and maintain that control. Since 2001, Ms. Pagano has worked in the financial services industry, and recently established Pagano LTC (www.PaganoLTC.com), a firm which focuses exclusively on long-term care insurance planning. I get tremendous satisfaction from helping people maintain their quality of life and independence, she says. We can help them make this happen by protecting them against the financially and emotionally devastating effects of incapacity. She utilizes For the Record as an integral part of the help she provides her clients. Ms. Pagano and her husband Ralph live in Phoenix, Arizona. Her husband is a physician with Arizona Medical Clinic, LTD, where he specializes in neurology. They have two adult children.

I have to admit I am not the most organized person in the world. I have papers in a small safe, in my sideboard, in plastic containers and recently,I had company coming so papers were stuffed in a bag and put under my bed.

This book is a must have for everyone. I know everyone says that about a book but this is the true thing. You can organized every thing and have it all in one book. Why someone has not thought of this before is a mystery to me.

It will help you and your family if something should ever happen to you or a loved one. Here are just a few of the things you will find.

1. Personal and Family Information
2. Medical Issues
3. Insurance
4. Legal Information / Document Locator
5. Employment
6. Income / Expenses
7. Financial Assets / Liabilities
8. Retirement Accounts
9. Final Arrangements
10. Important Telephone Numbers
11. Personal Property Inventory

The Appendix covers so much more that you will just have to buy this book to see. I think at one point in our lives we have thought of some of the things the author has mentioned. It was a learning experience for me, and I will use this book.

I can't stress enough how important it is to buy this book. You won't be disapointed if you do.

Ricki Pagano, I will start soon to fill the pages. I can't thank you enough. The trash men won't be happy campers though.

"I would like to thank you for the organizational book "For the Record". We were in process of revising our Wills and Trust. We needed something that would help us organize our documents and while surfing the internet I came across your book. Not only was it easy to use but when I met with my attorney I had this one book "For the Record" with all of our information. My attorney was quite impressed, and by having all of the information readily available, it saved us time and money with the attorney."

I work with many older adults who will find this product extremely beneficial in helping them organize their lives in such a way that will make it very easy for them to access important documents, records and so forth.
In addition, "For The Record" has a very enticing format, is self-explanatory and the forms are quite easy to read and use.
In the past, I have seen similar products that are priced much higher, so "For The Record" appears to be the best value on the market in my experience. I plan to recommend "For The Record" to a number of my patients, and to anyone who needs to get their life affairs in better order.

In general, I agree everyone should have their personal information written down somewhere for their families to have in case of an emergency but I don't believe this is the organizer to do it with. The style is outdated and oriented toward the "pre-internet" days.

It is oriented towards someone that only has one bank, in a brick building with one checking and one savings account, one insurance carrier with a local office you go to, one investment manager with a local office... These days both my personal banks are online, my insurance is online, (two different companies) , I have more than one life insurance policy, my investment portfolio is online without a broker... "For The Record" does not meet my needs for keeping my personal information in one place.

This organizer is not a complete waste of money but could be a MUCH better tool if it was updated. I tossed it into the trash shortly after receiving it in the mail.

It's a good book and very necessary. I lost my first husband and didn't have any of his information memorized or at hand when this happened. Now I know the importance of having all this stuff. Only issue is that it doesn't ask for more than one spouse info and I also want the personal information for my father and mother. It's actually something important and needed when you have custody of your siblings.

Title: For the Record
Author: Ricki Pagano
Publisher: Five Star Publications
ISBN: 978-1-58985-058-3

"If you take the time to fill out every page, you'll have a handy reference book that will allow you and your loved ones to find important information easily and quickly. That should take some of the stress out of emergencies," Ricki Pagano writes in the beginning of her book, "For the Record - A Personal Facts & Document Organizer."

At sixty-eight pages, this letter-sized paperback book is targeted toward readers wanting to keep all personal information, documents, and facts in one place. Simplistic in format, the font is large enough that the book would be welcomed for senior citizens or those having trouble seeing to easily fill out.

Divided into eleven sections, pages are dedicated to personal and family information, medical, insurance, legal information, employment, income / expenses, financial assets / liabilities, retirement accounts, final arrangements, important telephone numbers, and personal property inventory. The appendix includes listings of tax and other records to keep, contact information to replace lost or missing documents, and how to request a birth certificate.

With plenty of lines and spacing to hand-write in information, one can list generic data of date of birth to passport or driver's license number, surgical history and pharmaceutical addresses, location of a will, voter registration, or motor vehicle titles, life insurance face and cash values, credit card expiration dates, burial wishes including casket request, and phone numbers for sprinkler repair or veterinary care.
