All of the requisite forms forensic psychologists need in one compact volume — a crucial time-saver for mental health professionals in today's complex legal system

From child custody evaluations to domestic violence situations to competency hearings, the legal system invariably requires accurate documentation provided by mental health professionals. Knowing what paperwork is necessary in a given case and putting it all in order can often be a daunting, time-consuming task. Now Theodore Blau, a pioneer in forensic psychology and the former president of the American Psychological Association, has compiled the most useful forms in one handy sourcebook — allowing mental health professionals to do their best professional work on the cases at hand.

Providing blank, copy-ready forms—as well as completed versions of each form for reference—The Forensic Documentation Sourcebook helps prevent the type of errors and omissions that opposing counsels look for in court—errors that can lead to embarrassment and criticism at best, or criminal charges and civil lawsuits at worst.

What's inside:

  • Blank forms, as well as a disk for customized form completion with standard word-processing programs
  • Completed copies of key forms illustrating the type of information required—important insurance against the errors and omissions opposing legal counsels can use to question credibility or qualifications
  • Clear, concise explanations of the purpose of each form—including when it should be used, with whom, and at what point in the examination

5.0 out of 5 stars Virtual Secretary, April 30, 2000
This review is from: The Forensic Documentation Sourcebook: A Comprehensive Collection of Forms and Records for Forensic Mental Health Practice (Paperback)
Ok, so it doesn't take dictation. However, this book saved 100's of hours of work for me, in addition to raising procedural issues I'd not considered. A book of forms and organizers. Sound tedious? I would be if you had to create them all yourself. Book includes a CD with all of the forms copy-ready on it. Within Microsoft Word, I was able to redesign them to suit my practice, adapt them to my letterhead and put together packets of forms for different purposes. I hate office work. That is not why I entered forensic psychology. If you hate it too, buy this book.

5.0 out of 5 stars A must have for Forensic folks, January 9, 2007

This book is invaluable for anyone beginning a forensic psych practice. All the forms are easily adapted with your letterhead and are provided on disc. I have updated my contracts but other than that, still use most of the forms. I present a very professional file and am readily prepared for court. My disc did not work and after repeated attempts eventually went directly to the publisher. I was then sent an email attachment of the disc. It would have been very costly on many levels to have hired an attorney or to make errors on forms, or to not be aware of forms that were needed. Well worth the small amount for the wealth of aid to your practice.

for anyone who has given any thought to getting or staying organized while conducting forensic services, they've already generated most of these forms. i didn't find all that much in the way of direct aids to the practice of forensics. probably worth the purchase, used.
