For All Your Failure Analysis and HPI/CPI Investigation Needs . . .
This book is the best available resource for conducting industrial accident investigations. The processes and techniques can be applied for equal success on Root Cause Failure Analysis (RCFA) for catastrophic equipment failures. Truly, required reading for the sincere problem solver. Enough said.
