Health Care Without Medicare is a new practice manual for community-based care management. It describes the author's model for assisting frail elders, the physically disabled and the chronically ill to acheive their most often stated goals--to preserve their independence and their life savings. Community LifeCare Planning is described as the strategic use of personal, family and community resources to prevent hospitalization and nursing home placement among those most at risk--those with chronic health problems. The book was written collaboratively by two medical doctors, an estate planning attorney, a financial/retirement planner, three medical social workers (one of whom is a long-term care financing specialist) and two RNs. One of the contributors was recently named "Case Manager of the Year" by the Case Management Society of America. The book was written for nurses, social workers, therapists, lawyers, finance specialists, long-term or "geriatric" care managers, students and family caregivers. It is thoroughly researched and clearly written, with forms, indices and references that clarify the Community LifeCare Planning model. It is a manual for interdisciplinary team care management and resource management.
