I've only had this adapter for a little while but I can already give a little feedback for people looking to purchase.

First of all, it seems to focus on infinity just fine. I've seen where not all adapters are capable of this. The image quality is decent, though that has a lot to do with the lens/tilt combination too, I'm sure. Various lenses are likely to give slightly different looks.

The locking screws work fine. There are two of them opposite each other around the barrel of the body. My version is tight enough that I can leave them unlocked all of the time. Maybe over time it will loosen up and necessitate using the screws, but not yet.

The only troublesome thing that I've found is that there doesn't seem to be a way to lock the lens onto the adapter. The lens twists around snuggly (for now) but never locks into place (the adapter itself DOES lock on to the camera body like a normal lens would). I'm not sure if I have a defective unit or if they are just designed that way. I can see being a little uneasy with the unit if you have an expensive piece of glass on it. Luckily I'm using my old glass anyway so it's not a big deal for me.

UPDATE: I've used a LensBaby on my DSLR and it should be noted that this adapter doesn't tilt as much as the LensBaby does. That means that you can't get quite the drastic out of focus areas that you can get with the LensBaby. That could be good or bad, depending on your style. This adapter gives pretty good out of focus areas if you use it on the long-plane of the image area but the short plane gives very little in the frame that is out of focus.
