Weak on pre-op considerations, December 18, 1999

As a surgeon performing refractive procedures for over 20 years, I was certainly impressed with the surgical techniques described by Dr. Pallikaris, which in fact borrow from much of my own research. He has certainly learned much, and I am impressed with his progress to date.
However, Dr. Pallikaris and his colleagues have left a rather large gap in their coverage of pre-operative considerations, such as the need to check for scotopic pupil size, pre-existing dry eye conditions, and the presence of exphorias and extropias. He has also not discussed the increasingly higher risks of the Lasik procedure in high myopic corrections (6 diopters and above).
It's a shame that both surgeons and patients end up learning more about pre-operative considerations from bulletin boards such as surgicaleyes and eyeknowwhy than from medical treatises such as this, often too late to prevent permanent optical distortions.
