Malem recommends using this item on at least 6 year olds, but our (quite a bit younger than 6) son was completely ready to stay dry at night, but just clearly had trouble waking up to go. I figured, if you can definitely see that your child is ready and willing, but just needs some extra help, why wait until 6 years old when the habit is much harder to break if there is a good and safe help device? We made it a fun and exciting experience with lots of encouragement, smiles and no pressure - just like regular potty training. We showed him how the device worked and explained what to do and of course, met him in his room immediately when we heard the alarm go off at night. Malem sends a good pamphlet with really good tips and advice on how to make it work best, so we basically followed what they said. Within 4 days he got up completely on his own without going a drop in his undies, then he had a couple of nights with small accidents and now he has gone 3 more days without going at all. This device WORKS and is 100% worth the money! I really have no clue why Malem recommends waiting until 6 years old since there are many children under 6 that have the capacity and desire, but just need the extra help, but I can only say that it definitely worked on my child and I would heartily recommend it for anyone!
** UPDATE: He is completely night trained and it took just a little over 1 week. He has been completely dry now for almost 2 weeks!

No better pee pee alarm, February 18, 2005

Alarm assembly mounts with safety pin
We have been using the alarm for about 5 weeks now and our 4 year old boy has noticeably improved his ability to hold his bladder throughout the night. Our son is extremely difficult to wake up and the first couple weeks he slept through the alarm, while I woke up. It was not until week 3 that he started waking up after the first few drops of urine. He probably would have been completely conditioned by now if I hadn't anticipated the nightly 02:00 am wake up alarm and spent a week getting him up before a bed wetting, thus potentially cheating him out of some of the training. (You just can't help but wanting to avoid changing sheets and PJs several times a night.) Finally on week 5 we are seeing so much improvement, that we're considering letting him sleep without it.
The Malem alarm is priced comparably to the competition, however, it is clearly the best product in it's class. The alarm system is easy to use, cleans easily and is so durable that it survived being mistakenly put in the washing machine. The end piece that clips to his underwear is very intelligently designed, cleans/dries easily and fits comfortably. The product shipped with two sets of batteries and we are still on the first set. The only shortcoming, in my opinion, is that the alarm assembly fastens to clothing with a safety pin. While we have had no accidents with this pin, safety pins make me especially nervous when placed near the neck. I would give this product 5 stars if not for my perception of safety pins.

Excellent - problem solved!, December 4, 2008

My almost seven year old daughter has never had a dry night. Although her pediatrician reassured me that she would grow out of it and I never made her feel the least bit ashamed, I knew she still felt embarassed and frustrated that she needed a pull up at night. I had purchased a different alarm first that was impractical - you had to sew velcro pockets on her underwear and slip the alarm in the pocket. The pocket was uncomfortable and quickly started to come off in the wash. I found the Malem on Amazon and was immediately impressed by the simple and safe design. The alarm is piercing and woke us both up immediately. I helped her to the bathroom and to change her panties. I had her wear a pullup over the panties. (I don't know why people feel that changing the sheets are part of this - the alarm will work whether you use a pull up or not.) First two nights: woke up twice. Next two nights: woke up once. After that, dry every night. I don't understand how it works but I am thrilled.
