This book taught me a couple of valuable techniques, but on the whole, I found it lacking. It focuses too heavily on reductive techniques to solve every cosmetic and functional problem. Grafts are rarely mentioned. I was interested in the "incision only tip refinement technique" and would use it, but aside from this, most of the other techniques are either not unique or overly reductive. I liked the "tissue clay" technique for filling slight irregularities, and although the titanium reconstruction is probably quite effective, I think it is a bit of an overkill, especially as he describes using it to rotate the nose in primary rhinoplasty patients.
To rotate the nose he advocates the antiquated technique of cephalic trim combined with resection of the caudal border of the upper lateral cartilage and relies on scar contracture to rotate the nose in the postoperative period. This technique is too unpredictable in my opinion, especially since collumelar struts and lateral crural overlay are more effective, less destructive, and do not rely on prediction.
Alternatively, he recommends titanium chain inserted through transfixion and transphenoidal incision when more rotation is needed. I think this is extreme, especially for a primary case.
