This book is an excellent guide for students, residents and practitioners of neurohospitalist medicine - a rapidly developing model of neurology practice. This practical, concise book gives a superb overview of neurological issues commonly faced in an in-patient service, be it on the primary neurology service, consultation service or intensive care service. The book is well organized and in addition to providing sound advice for the diagnosis and management of neurological issues, this importance work also engages ethical and organizational aspects of neurohospitalist practice. The days of the neurologist shuttling back and forth between a tightly booked out-patient clinic and the hospital for emergency consults are numbered, and increasingly we are seeing a well justified rise in hospital based neurologists. It is likely that these neurohospitalists will provide more efficient and higher quality care to the hospitalized higher intensity patient just as their medical hospitalist brethren have demonstrated in internal medicine and pediatrics. Many argue that much (perhaps too much) of neurology residency training occurs in an in-patient setting and therefore all trainee's are qualified to be neurohospitalists, and that is to some extent correct. However, the organizational, economic, patient safety, pragmatic and quality assurance aspects of the practice of medicine are often not adequately addressed during residency training; neurohospitalists are uniquely positioned to develop demonstratively effective approaches to the neurologically ill hospitalized patient and that takes additional training and a new culture of practice and outcomes research. The authors are pioneers in this newest subspecialty of neurology and their guidance should be heeded. Highly recommended!
