There is a concern among many buyers that this warranty must be registered within 10 days of the product purchase but that is no longer true. See here.

However also note that when you mail the copy that has to go to Nikon, they do not...

Caution: May not be valid for you.
Product Description does not state that this "Agreement" must be mailed within 10 days of camera purchase to be valid. I found this out upon receiving the Extended Service Coverage Agreement in the mail. Technically Nikon does not have to honor this Coverage Agreement, since I purchased the camera more than 10 days ago.

However also note that when you mail the copy that has to go to Nikon, they do not acknowledge receiving it. So keep your copy safely and make sure you send it by certified mail etc and keep a record of delivery.

I've had an ESC on both my D200 and D300. It has definitely been a worthwhile investment (at several times the cost of this deal). When the rubber grip started to separate on my 200, I sent it in and, for the price of shipping one-way, Nikon completely replaced the grips, cleaned the sensor and addressed a few other small issues I had. The repairs would have run over $100 (including labor, of course) but I got it done for $20 or so. The sensor cleaning was an added bonus. There are, of course, things you can do that WON'T be covered, but given the incredible expense of some repairs (sensor issues (banding, pixel failure, etc) can be many hundreds of dollars), an ESC is worth the piece of mind. At under ten for this ESC, it's basically a steal.

And as another reviewer stated, an ESC can be purchased and registered anytime within Nikon's manufacturer warranty (one year). I've had conversations with service reps with each of my previous ESC purchases and they've been quite clear that the 10-day limit is "misleading." It was meant that you send the ESC in within 10-days of purchasing... the ESC, not the camera. Also, Do save the card since Nikon will not verify they received the ESC (as another reviewer stated). I even write the date I shipped it somewhere on the card.

I received the extended coverage before I received the camer. After I sent in the information on the camera i sent in my extened warenty. As of this date I have not had to use it. The camera is a Nikon D-69. As of this date I am completly satisfied with the warenty considering the price I paid for it.
