Building upon the theoretical work of Ferenczi, Fairbairn, and Berliner, the author describes four basic relational patterns in the lives of abused children: the reliving of abusive relationships, either as victim or as perpetrator; identification with the aggressor; masochistic self-blame; and the seeking of object contact though sex or violence. The interweaving of these patterns creates what Dr. Prior calls "relational dilemmas." According to him, these four basic relational patterns are held in place by the child's profound fear of falling into primitive states of unrelatedness and consequent annihilation anxiety. For example, the abused child believes that victimization by or identification with the bad object, no matter how horrible that may be, is preferable to the psychic disintegration that complete nonrelatedness creates. Dilemmas of this nature tear apart the child's psyche, leading to unstable and tormented models of self, other, and relationship.

Object Relations in Severe Trauma provides sensitive understanding of childhood traumatization and a conceptual and technical framework for the treatment of patients--both children and adults--who have suffered from it.

Object Relations in Severe Trauma provides the unusual amalgamation of solid scholarship, sound theoretical development, and detailed case examples of treatment of the seriously disturbed and traumatized child. Drawing from both his psychotherapeutic treatment of seriously disturbed boys who suffered multiple forms of neglect and abuse and from the work of Fairbain, Berliner, and Ekstein, Prior delineates an object relational theory of the consequences of the sexual traumatization of children that has implications for therapy. Students and practitioners will benefit from the illustrations of the psychological response to trauma in terms of the repetition of abusive patterns of relationship, identification with the aggressor, self-blame, and the seeking of object contact through sexuality or violence. This is probably one of the most valuable books for clinicians who provide therapy to sexually abused children from multi-problem families. (Judith L. Allpert )

About the Author

Stephen Prior holds doctorates in both philosophy and psychology. After receiving his degree in psychology, he worked extensively with seriously disturbed children. Currently he is in private practice with adults and children in the greater Boston area.

As a therapist who has worked with severely traumatized children, I have found Dr. Prior's book to be extremely helpful in understanding the complex forces that drive these children to relive their original trauma again and again. Dr. Prior integrates relevant object relations theory, and takes us inside the sexually abused child's violent, tormented world where so much conflict centers on being able to trust and believe in him/herself. Using the case example of Damon, Dr. Prior shows us how to provide sharp, effective treatment geared toward restoring the sexually abused child's sense of safety and trust, so he/she can risk coming out of his/her defensive shell, and give up the victim/persecutor role as a mode of relating to others.

Overall this book is written in a clear, practical way, and provides a thoughtful exploration into the psyches of severely traumatized children. Dr. Prior's spirit shines through in his work, and the book encouraged me to think more deeply about how terrifying it is for such a child to live on the edge of annihilation anxiety, and how to sensitively address this disturbing phenomenon. I highly recommend it as a steady guide and souce of inspiration to all therapists who embark on the difficult journey into the traumatized child's dangerous object world, and wish to come out on the other side reaping its great rewards.

Prior's book concisely shares his observations about what distinguishes children who have been repeatedly, severely abused from those who experience less severe or frequent abuse. He specifically focuses on the beliefs these children come to have about themelves and how they relate to others, from an object relations viewpoint. The book's only downfall is is it's lack of information how to treat these children, once you understand the relational dillemas they face.

very informative and enlightening book for understanding and treating survivors of child sexual abuse. the conceptualizations are helpful, and powerful drawings are used to make concepts clear and give insight into the pervasive impact of sexual abuse. this is a well-written text with very practical treatment ideas.
