SUBJECT: Bone marrow transplantation was first successfully carried out in the 1960s and was initially performed for the treatment of acute leukeamia. However, bone marrow transplantation is now performed for a wide range of conditions including inherited bone marrow disorders such as severe combined immunodeficiency, chronic granulomatous disease and osteopetrosis as well as malignant bone marrow disorders including leukemia and myeloma. Transplantation may take the form of whole marrow from an unrelated donor or stem cells mobilised from the patient''s own bone marrow. More recently transplants from cord blood have also been used as a source of haematopietic stem cells.NEED: In this field, a range of paediatric and adult bone marrow transplanations are performed for a wide variety of disorders. The underlying condition of the patients and the immuno suppression used in bone marrow transplantation predispose the patients to a wide range of infections. In addition, other complications may arise as a result of transplantation, particularly graft versus host disease which requires a careful balance between suppressing the condition and trying to reduce the risk of infection at the same time.The management of patients requiring bone marrow and stem cell transplantation involves the input of large numbers of health care professionals and there is now a need for a practical, concise manual to guide all those working in this field. As an ID doc who seems BMT, I was looking for a "BMT for Dummies" book to make sure my fundamentals were sound, and to have as a resource for the ID fellows. This fit the bill reasonably well, with definitions for a lot of basic concepts and procedures. It gives the basics of most aspects, from indications, the mechanics of matching and transplanting, complications, and infections.
I've given several chapters to the fellows, who liked them and felt that it filled in important gaps in basic knowledge. I haven't gotten feedback whether the onc fellows like it.
This small book does not pretend to have the true depth needed for a real practicioner.
The target audiences I picture might be the following:
- Consultants who need to understand the process better
- Heme/onc fellows beginning their rotations
- Nurses, NPs, PAs who are just beginning to work with HSCT patients and need a place to start.
The 4/5 rating comes from a general sense of adequacy, but I can't find many specific complaints. It needs updating in cord blood transplants. The ID chapters seem a bit short, but I'm biased.
I felt this was a good supplement to Dowden et al. "Transplant Infections" to round out the BMT ID experience.
