This text is a good overview of the basic theoretical and practical implications of questionnaire writing for second language research, although it is slightly outdated in terms of computer applications available for questionnaire writing (which is to be expected with a 2003 publication date). I used it this fall in an undergraduate research class and the students found it accessible and informative. I would use it again.
