Sanus MC1A
Sanus products are great. I have installed 3 mounts from 26" ceiling to one holding a 65" plasma. They contain all the parts and more, good instructions and are very sturdy. Highly reccomend.

To lightweight
Used for 40 inch TV. Will not stay level, need to brace TV. Not enough vertical adjustment.

This was very easy to install and is very sturdy. You can use the Sanus adapter to configure it for wider hole placements.
