Book Description

March 2000
Healthcare has never been more splintered and impersonal. With insurance companies and computers wielding an inordinate amount of power over the medical profession, there is a universal need to take responsibility for one's own health.

Designed by a doctor to help individuals and families, THE SAVARD HEALTH RECORD is an interactive guide to compiling copies of your original medical records and other important health documents. Organized in a handy, 3-ring binder system, THE SAVARD HEALTH RECORD makes it easy to document the health history of an individual or an entire family. From maintaining immunization records to documenting prescriptions to tracking test results, here is the only guide you'll ever need to manage complete-life care.

From the Publisher

Doctors have less time to spend with each patient they see, insurance companies make medical decisions, and patients have lost trust in the health care system. Patients feel the need to be responsible for their own health. THE SAVARD HEALTH RECORD is an easy method of organizing and tracking your medical records and other important health information. People should have their own complete medical file, including copies of all blood tests, lists of allergies and medications, immunization history, eyeglass prescriptions, and the like. If you have to see a specialist or a new doctor on short notice or have to switch health plans, there may be a delay in gaining access to your medical files, and reports and test results can be lost or misfiled. Keeping track of your own health information makes sense. *Add and remove pages with the flexible 3-ring format *Dozens of helpful, easy-to-use charts (health journal, family medical history, vaccines and shots record) *Store prescription information, insurance cards, doctors' business cards, and appointment cards in the folders and pockets provided *Use the calendar to keep track of upcoming tests, examinations, needed immunizations, and medical refills

From the Inside Flap

You have in your hands the key to getting the finest health care possible. THE SAVARD HEALTH RECORD is a simple six-step system for putting together your own complete, accurate, and continually-updated medical chart. THE SAVARD HEALTH RECORD shows you how to take an active role in preventing and managing your health problems.

HERE'S WHAT'S INCLUDED-- *User's Manual which explains everything you need to know about your rights (and responsibilities) as a patient to obtain your medical information: how to accurately write down your medical history, gather your information from doctors and hospitals, understand the results of most standard tests, and develop a personal program to maintain your good health. *Glossary of Common Medical Conditions and Terms to help you understand test results and other medical information. *19 Forms to help you organize and keep track of your health care, such as the Family Medical History Tree, Emergency Health Information sheet, an Office Visit form, Medications, Vitamins, Supplements, and Herbs Log, Test and Procedures Log, and many more useful charts and journals. *Envelopes to hold everything from your Living Will to prescription information sheets. 5.0 out of 5 stars Fantastic Record Book!, April 23, 2000
By
This review is from: The Savard Health Record: A Six-Step System for Managing Your Healthcare (Hardcover)
What a fantastic record book! The "Savard Health Record" provides a solution to a problem my husband and I encountered approximately two years ago. After my father passed away, my mentally handicapped brother and sister came to live with us. Among our many new responsibilities was meeting their health-care needs. When I took my brother and sister for their first physical, I wondered how I was going to keep track of all that was happening to them medically? I could barely remember most things about my own health, not to mention health histories, of my husband, my children, and my brother and sister. I felt that I would just have to count on their doctors to have all of the information they needed. Then I heard about the "Savard Health Record". What a fantastic tool for keeping all important medical information organized for my family and especially my brother and sister! I also appreciate the peace of mind that I'll know when my brother and sister are in someone else's care when I'm away. I can leave their health records with the relative or friend who is caring for them and know that the caregiver will have all of the information needed for them in case of a medical emergency. "The Savard Health Record" is a great tool which gives us the means to take control of our health care and of the health care of those we love, especially those who don't have the abilbty to do this for themselves.

5.0 out of 5 stars Excellent Resource For Patients, April 25, 2000

Dr. Savard has put together a simple binder-formatted book that makes it easy to get organized when it comes to your health records.

As someone with hypothyroidism, a chronic illness that requires frequent blood tests, it's really critical for me to stay organized. This format could be of help to those of us who need to stay on top of our tests, results, and health information in order to live well.

I recommend this to anyone with a chronic condition or disease, or a family's health records to manage.

5.0 out of 5 stars Life-Saving System, April 3, 2000
This review is from: The Savard Health Record: A Six-Step System for Managing Your Healthcare (Hardcover)
A few years ago I never gave it a thought to collect, store and actually learn how to read my test results. The current times have changed that. We moved a few years ago, and I like so many others thought that my medical records moved right along with me to my new doctor. Wrong. I developed a medical problem where I needed to refer to old medical records. It took months and alot of grief to have my records transferred to my new doctor, but I was persistant and finally, they were sent. I can't begin to tell you the difference it has made for me to have a hard copy of all my test results, doctors summaries, medications, allergies, etc., all filed in my "The Savard Health Record." The specialists I see have been impressed with my record keeping system. I'm now prepared for every office visit by taking my Health Record book with me. I have my office visit form filled out with my questions and concerns. Physician's can no longer give us the time they once did. Practices are much too large for any physician to know the history of each patient. I am confident, and no longer feel powerless, in helping to make the correct decisions about my health and health care. "The Savard Health Record" includes an emergency information wallet card that I carry along with my insurance card. What good is insurance if one's vital health information is not known or forgotten in an emergency?
