Stephen L. Isaacs, J.D. is a partner in Isaacs/Jellinek, a San Francisco-based consulting firm, and president of Health Policy Associates, Inc. 

James R. Knickman, Ph.D., is vice president for research and evaluation at the Robert Wood Johnson Foundation.
