What's in the box: Exactly what you see in the picture, Block and D-Ring. You'll need to purchase a hitch pin separately (looks a 5/8" hole).
Thoughts: I posted some pics so you can see the workmanship as well as the fit on my Tacoma. This is not tubed metal; it's casted and is solid (weighs about 9lbs). D-Ring fits perfectly within the block... no wiggle yet can freely spin. No paint chips either. Cannot wait to write a follow-up after my first use and am confident this will be a vital piece of my recovery kit for a long time.
UPDATE: I confirmed this uses a 5/8" hitch pin.
The pictures show a smooth, machine-like product. It is cast and ground, poorly at that. To Amazon's credit, they made returning the first one easy. Unfortunately, I'll be taking advantage of that service again.
It seems that the negative reviews about size variations are from a few years ago, and based on the one I received, they seem to have the size right. The main block on mine (drawbar?) measures 1.90 inches wide and 1.97 inches tall. My 4Runner hitch measures 2.08 inches wide and tall, so there is about 1/8" of extra space between the receiver and the drawbar. Without a pin, there is quite a bit of play, but with the pin inserted, it's pretty snug.
As reviewer Marcus noted, on the 4Runner, the receiver pin isn't very far back, so this hitch will stick out quite a bit. Since I don't plan on using it regularly, I am keeping mine inside the 4Runner, so I'm not going to bother drilling an extra hole.
The metal surface is not smooth, like the Amazon product photo. The powder coated surface looks like a really heavy orange peel / hammered paint texture. But the powder coating feels very thick and heavy, and I don't care about how smooth it is, so it's good enough for me.
Overall, it seems like it is very good quality.

We were able to pull several large (1ft diameter, 4ft long) logs around a field with no issue. The final task was to pull down a 30" diameter tree trunk, about 15ft long, down and then across the field to the burn pile. The product worked well for this task and had no visible damage after use. I've also seen people use this to pull people out of trouble spots multiple times while recreationally off-roading. It has always held up, and its solid base is very inspiring (most others on the market are hollow). I think we purchased a quality product that will stand up to years of abuse on the farm.
