Despite the fact that modern low-dose oral contraceptives are very seldom associated with serious adverse effects, knowledge of the contraindications for oral contraceptive use is particularly important because of the many misconceptions that still exist and which are largely historical in origin. Reports as long ago as the late 1960s associated oral contraceptives with an increased risk of thromboembolic complications, myocardial infarction and stroke, as well as other contraindications. Most subsequent epidemiological studies have shown up the methodological shortcomings of the original population-based studies, which usually did not take account of pre-disposing risk factors and other relevant considerations. In spite of this, many women using the Pill today, as well as a significant number of physicians prescribing it, still believe that the risks involved with oral contraceptive use are substantial. This is reflected by the extensive list of contraindications to the Pill in the literature, and in the labelling of oral contraceptives. This has led to the Pill being withheld from some women who most probably could have benefited from it safely, whilst other women have been exposed to the Pill unwisely, putting them at some increased risk of exacerbating certain medical conditions. Critically reviewing a large amount of literature and data, this book reconsiders the safety profile of modern low-dose oral contraceptives. The authors use the following basis for their evaluation: the characteristics of the condition - definition, major clinical symptoms, prevalence, severity, pathogenesis and major risk factors; the relationship with oral contraceptive use - epidemiology and mechanism of action; and the contraindication status of the condition. This book should provide a useful reference for physicians and others who want to achieve a balanced understanding of the proper use of the modern, low-dose oral contraceptive. The data presented should make a constructive contribution to the current debate on this subject and be a practical aid to clinicians in their daily practice.
