This is a wonderful book. It was given to me to review by one of the charge nurses where I work--a long term care facility in a small town in Iowa. This text not only gives very good instruction on documentation, it is organized well. Each cover includes a quick reference guide to assist with finding specific areas of concern. It is well written and includes text boxes with more specifics and concrete examples. I have recommended it to my Director of Nursing Services as a resource that could be used in the other 2 nursing facilities in the corporation. An excellent reference text with easy to find entries! I would highly recommend it to anyone who wants a ready reference.

This book is old (over 10 years) so has no current information. Laws change quickly which invalidates the content. Simplistically written and too basic

I recommend this book for nurses who have been out of the hospital setting for awhile, to review documentation skills. I currently work in a school setting. It is good for returning to work part or full time. It helped me out when I returned to work every other weekend in a long term care facility.
