Let me just cut to the chase. I'm an assiduous step-1 studier and I own pretty much every resource from FA, BRS, Goljan, QBooks, QBanks, Microcards, Lange pharm cards, many of of the HYs and many of the DejaReviews. Underground Clinical Vignettes - Anatomy was FANTASTIC. It's an incredibly affable read and takes about 5 hours to finish (~10 to appreciate). It covers the CLINICALLY relevant anatomy that shows up on the exam. For example, a lump over the buttocks in a new born --> petit triangle hernia, then it gives you the boundaries (iliac crest inferiorly, anterior aspect of latissimus dorsi posteriorly and posterior aspect of external oblique muscle anteriorly). And that's just one example. The USMLE is all vignettes, and people who score well do so because they know key words and things to look for, not just because they have mere shelf knowledge. I believe this book will have earned me at least 3 or 4 additional points on the exam. Read it once, then before the exam, give it another half hour review --> twice total.
