can you access all controls (headphone jack) while in the case?

Yes, all outlets are accessible with the case on. The Kindle fits inside tightly so you don't have to worry about it coming out or the elastic straps breaking like other ones. You'll like it.

Yes, it's quite easy to plug in my headphones with this case. I even switch between headphones with a straight pin and one that elbows to the side and both fit well with this case.

Yes, you can get to all the controls while in the case, but I believe the cover has to be open because when you close the cover it puts the Kindle to sleep.

Yes I am able to access all jacks including headphones, charging, etc. All good.
