I ordered this form thinking it would look the same as the picture. It was not! I have ordered it from Amazon before, and it looked like the picture. Apparently, Adams has changed it and it no longer looks like the picture. It is not set up the way we would like, but it will do.

I've been buying these forms for years and they have changed them since my last order. There is no place to put the date, and the disclaimer in the middle of the sheet is unnecessary and takes up to much usable space. very unhappy and want the old ones back!
