Having experienced wrist strain from regularly using a normal mouse, I was excited to finally purchase an ergonomic mouse. Between the Penguin and the Evoluent brand, I decided to go with the Pengiun because I felt it would be a more natural fit for a relaxed hand. And yes, I was pleased with the results. It is much more comfortable to use than a normal flat mouse, and I found the control was not that hard to get used to.

However...

Two days after receiving and using the mouse (approximately 8 hours of use), I went to use the mouse and noticed that the top button was stuck in a depressed state. I observed that the scroll wheel was titled up slightly, which caused friction on the button. It turns out that the scroll wheel had broken off its axis with the rotation sensor, meaning that the scroll function would never work again. Further inspection revealed that the scroll wheel was directly attached to the rotation sensor by a 1/16th inch rod of plastic sticking out from the center of the wheel. Mind you, the scroll wheel also acts as a third button, so that you can click by depressing the wheel with your finger. The very act of using the button, which is a part normal use, rocks the wheel back and forth against the connection to the sensor, eventually snapping the plastic rod like a small twig.

For how much this mouse costs, I am disappointed that this design flaw was overlooked, and I do hope that this is fixed with future versions of the device. Simply embedding a small metal rod into the center of the scroll wheel would have prevented this from happening.

This is my first purchase from Posturite and my last.

I do like the ergo design. I got used to the movement requirements with this style of mouse. Sizing is going to be personal preference I think. I like the Large, as specified for my hand size, but I could see it being too big for someone of the same size.
For my job, I do a lot of 3D modeling which leads to my ultimate complaint:
The middle click (scroll wheel) just takes too much force and travel to activate. I think that all of the buttons take a little too much force to activate than is necessary, but the middle one is the deal-breaker for why I'm not constantly using it (it takes about 2X the force of the other 2). It's sitting on my desk next to my main mouse (also a vertical-style, but not 90-degrees like this one, more like 60-70 deg). If I'm in heavy office-mode (doing presentations or whatever that doesn't require the middle-click), I'll make use of the mouse; but it became apparent very quickly that it is not suited for modeling work where one depresses the middle-click to rotate the view (I'm constantly running 2 programs that function this way). The other buttons need the same kind of attention. If the buttons had a light-touch activation, this mouse would be king (for me).
Because it takes a relatively excessive force, it always disengages the middle-click function while the button is still down. As a result, you're constantly death-gripping (again, relatively) the button to prevent this from happening. This becomes very annoying very quickly.
I would recommend it for someone who doesn't care about the middle-click.

This is one instance where seeing is believing.
I started out with the Evoluent Vertical Mouse 4 (same idea/principle for carpal tunnel symptoms).
In my opinion, this mouse is a step up in almost every way, at least for me.

Let me explain what's better:

1) Click free gripping- the whole handle is blessedly click free. No worries about accidental click pollution- although it does have fully useful button capability (takes a little getting used to, but not too hard to adjust).
I say useful because I don't need more than 4 buttons on a mouse. I just want to move a cursor around and click, not operate a PS3 game.

2) Place to rest your hand- The biggest problem I had with the Evoluent mouse was, where to lay down my hand when NOT clicking. That is, if you don't want to levitate it in the air for 9 hours.
Unfortunately with Evoluent, your hand being on its side, you're forced to rest the corner of your wrist-bone on the desk, if you want to stay attached to your mouse "at the ready". Maybe you can get used to this; but after a day of Evoluent I had new wrist pains I never had before. That was enough.
The Penguin Mouse has a big, wide comfortable base to put your hand, and you can also rest on the click-free top.

3) No 'Christmas ornament' material- Grippy material instead of that slippery metal stuff.

4) (And this is probably the biggest plus-) Left and Right hand switchable- If you have ever had carpal tunnel, you know switching hands is your first line of defense when your joints get tired.
Impossible with the Evoluent, but here, a flip in the front reverses the buttons, and it can be used with either hand.

I use this with both hands, and it works perfectly. The toggle switch changes scroll wheel direction and swaps the button functions so it is easy to use with either hand. This is a comfortable design if sized correctly. I find it easier to use than the Evoluent mouse, and standard mouse drivers will work with the Penguin. Be aware that if you are used to moving just your wrist when mousing, you will need to relearn. This design prevents wrist strain by making you move your whole forearm. This is a little difficult at first, but the payoff is reduced wrist discomfort.

For ergo correctness, hand and fingers should be in as neutral position as possible. If the mouse is too small, fingers need to flex more, eventually causing strain. The manufacturers website has a sizing diagram to help you order the right size.
