CS4 upgrade messes with file associations

I've had a horrible time getting CS4 working appropriately in my office - where we have three Adobe CS installations. The most recent problem: upgraded user from a working version of CS3 to CS4 on Vista - now she can't double-click on a file to open it. All file associations are screwy - even the icon for the files is changed. She either opens from File->Open or right clicks on the file and selects the appropriate executable from deep within the file associate dialog. Not finding any web references from others having this issue. Anyone else running into this?
