Carrie, to remove the book from Fire itself, long-tap it (tap-and-hold) and select the apropriate action from the menu. You will still see it in Cloud tab (and Carousel, I think), but that's the view into your library on Amazon servers.

To delete it from there (*forever*), go to http://amazon.com/myk page, find the book, hover over "Actions" button and select "Delete from library" or similar. When you next sync your Fire, if should disappear completely.
