The "Index" from the Menu button

If you type another letter, it goes to the first link starting with those TWO letters, etc. One of the best examples of this is the free, Kindle version of the ESV Bible.

My question is this: Is THAT index something that the Kindle, itself, can generate (perhaps relating to the indexing that the Kindle usually does) or is it strictly something that was prepared and included by the publisher of the book (i,e, by their establishing each and every link point to list)?

If the former is correct, then what must one do to be able to have the "Index" option appear on a book's "Menu" button? If it's the latter, however, how may one, given the necessary text (e.g. a different, public-domain version of the bible that isn't, already, so indexed), produce such an index to allow for easier navigation? (I do understand that this second option may not be something for most end-users to undertake, but I have many years of IT experience and would be willing and able to learn this process in order to make some of my more-frequently used, public-domain books easier to navigate.)

I see that I am the first reply to your post. Unfortunately, I do not have your answer either, but I have the identical question as you. I also discovered this with the ESV Bible. So, I have a few questions:
1: Other than the various Bibles (from various publishers) in which I have seen this Index in the Menu, have you found any other books that exhibit this behavior? Would you please name them?
2: Have you found out from other sources how to produce such an index when editing and formatting a Kindle book for publication? Please feel free to email me at editor7434@fastmail.net to correspond about this issue. I have a few tiny tidbits to share with you.
