It means how you want your Kindle books to be displayed on the screen. If you sort by title, they will appear alphabetically by book title; if you sort by author, the books will be arranged alphabetically by author. 'Most recent first' shows the books in the order they were last opened. I usually use this one when I am reading a few things at the same time and don't want to go paging through the display to find something.
