Really depends on which kindle you are talking about...

The cost of the device, plus whatever you usually spend on books a month.

Any of the Kindles, you have to purchase books for them, though there are plenty of free books available through sites such as ereaderiq and pixel of ink, as well as the Kindles having the ability to read books from your public library if it participates in the Overdrive system.

For the Fires (tablets), you would have to purchase apps or get free apps from the App Store or from places on the Internet that provide free app files. You'd also have to pay for (or rent) movies that you stream, unless you are a Prime member and can stream videos for free from Amazon.

For all of them (except for the 3G e-ink models where Amazon pays for the 3G), you need to have access to a wi-fi signal, whether it's a public wi-fi hotspot or a wi-fi network in your own house (you'd have to buy or rent a router, as well as paying an Internet Service Provider, which I'm assuming you already do since you are posting here).

Thank you, trying to decide if I would get one for the wife, and wondering if she would use it too.

That could mean anything, as "top of the line" is subjective. But I am assuming you mean one of the Fires. Folinlalala has answered your question, but if you need a data plan for a 4g Fire, it will depend on which one you choose.

You will also have to pay $50 for the first year of the 4G service (the price might be included in the total price of the Kindle; I'm not sure since I'm not in the market for that model so I haven't checked the product description). But after that first year, if you want to continue the 4G service, you will have to pay a monthly fee--those rates are determined by AT&T.

If all she is going to do with it is read books, than you don't need a Fire, just one of the regular eInk Kindles. less expensive, no fee for wireless.

The inner net is free.

It's getting access to the outer net where you really get gouged.
