My all time favorite "inner sleeve": Linda Ronstadt's album "Simple Dreams", with the picture of Linda with the orchid in her hair, peeking over her shoulder with those big, brown eyes.
(I have no idea what songs are actually on that album!)

Some of you have mentioned album covers that had a cool gimick (being in 3D, the working zipper, et cetera) and this makes me sad that when these are translated in to CD the effect us usually lost and relegated to just the flat image in the booklet. Also, new CDs are rarely very innovative like some albums used to be. Coolest thing I've seen lately is a Kanye West CD (not a Kanye fan, promise) that's done up in gold foil. Very cool looking cover.

I don't really have a particular favorite album cover, but I can list some that I like -

AC/DC - Back in Black or TNT
Rolling Stones - Let it Bleed or Some Girls
Beatles - Seargent Pepper
Supertramp - Breakfast in America
Van Halen - 1984
