It varies. Some have none, some have a moderate amount, and some have tons. Usually if they are more graphic sex than story, they are considered erotic romance, and if they are mainly graphic sex, with little to no story, they are considered erotica.

Please note: to be considered 'Romance' per the industry standard, they must have a Happy Ending, or Happy for Now ending.

You'd actually have to go into the thread to determine. Most of the time it's NOT spamming. Some readers like specific elements in stories. Or sometimes they've read a certain book, but can't remember the title, and name the thread with the most prevalent detail they can think of. In the OP, they usually clarify.

That. It's actually very rarely stealth spamming, and very obvious when it is. The majority of book requests are old books, books that are out of print, longterm authors. Rarely are they indies, or even books published in the last two years.

Eg "looking for book read long time don remember too much of it" - book was The Gilded Web by Mary Balogh, first published 1989 by Signet. An author with a very large portfolio.
"Need help finding a historical romance(warning: bodice ripper & graphic)" Thread has been running since Nov 17, no book yet, rec's go back to 1979 publishing date. Definitely not spam (or if it is, it's not good spam).
"Rebecca brandewyne please help!!!!" - Brandewyne has been writing since early 1980s. Thread is specifically asking about backlist books, and whether they're avail on kindle. No recs actually made.

That said, a few "it" books and authors have arisen through the threads, and get a heavy push/recommendation. Some have active authors who also post around the books - but the authors usually have a heavy history on the threads and have been welcomed. They *don't* push their books, but their fellow posters do. Examples Verdenius and Anders. And they are "allowed" (in fact encouraged) to respond to questions about their books. There are other authors too, and some who post under pseudonyms.
