I can be used without a belt. Just stretch it over your waistband.

The clip does work best with a belt, however I have used it to clip to a pocket and different things with okay results, I would not try jogging or anything strenuous without it clipped to a belt, but for normal to sedate movement it was fine.

It is a belt clip; however, I clip it to a shirt collar and it generally works well.

Pretty much; it is a tight slide-over clip.

The clip is tight and doesn't have a lot of stretchability so yes, in my experience, you need something fairly thin to clip it to. Hope that helps.

That's not necessarily true, but it depends on which activities one does with the clip attached. Myself, I only use the clip when i'm doing household chores and don't ever wear a belt doing those. But running, one probably should just hold it in their hands.
