I would like to buy the DS Brain training for one of my cousins who lives in Holland. Is it possible to change the language setting on it or does it have to be in english only?
Advice greatly appreciated.

Although there are language settings for the DS itself, there are none for the Brain Age game. Other versions of the game exist for Spanish, French and Japanese but they are completely other games and not just a game setting.

I was actually looking for exactly the same thing (Brain Age in Dutch). I found that there actually is a Dutch version available. It is called "Dr. Kawashima's Brain Training" and "Dr. Kawashima's - Meer Brain Training". I found these on www.bol.com.
