I ordered mine on-line. AT&T used the USPS to deliver to my office in a high-rise, but unlike Fed Ex and others, the USPS refused to look at my name on the building directory to locate my suite number (which AT&T omitted). While the card was bouncing around 2000 miles away in a Texas postal facility, AT&T refused to issue me a replacement without charge until the card was returned. Meanwhile, I couldn't get any help from the USPS.
So, then, I simply asked that the transaction and the contract be cancelled. AT&T refused to cancel until the card was returned, telling me to call back each day without any clear direction as to what AT&T would do if the card simply disappeared. Of course, each call on the service line took 20 minutes to connect.
