This book has essentially everything you need to know for most internal medicine stuff. It has great background info as well as treatment algorithms. I think there is some minor room for improvement on drug details but it usually is sufficient to find a treatment without having to look elsewhere. The new version expands on the old by adding in several topics that you see more in the outpatient setting (such as obgyn, ophtho, etc) as well as updating previous topics. Another great thing is that you can always add more pages yourself in between because of the way it is in the binder and therefore you can really customize it. Overall I think it is a great mix of being detailed and concise. I have heard multiple lectures from attending level physicians that essentially restate word for word what is in the book (and you look super smart when you already know it because you just read it in pocket medicine...).
You can always look at other sources such as dynamed, uptodate, etc to get into some of the finer details but overall no pocket reference even comes close to pocket medicine's usefulness and utility. I highly suggest this source to any physician or future physician.
