Poor build quality

This is a great idea and the product initially seemed to work well. The LEDs were not as bright as a flare is (or even a typical taillight) but I was happy enough with it given its battery life.
However there were two major problems, both related to build quality:
1/ First, one of the A/C charger's two prongs broke free, leaving it exposed in the outlet. I had to cut power to the outlet before I could remove it. This was not the result of excessive wear (I'm gentle with my electronics) and I had only plugged it in once.
2/ I was able to charge this with a standard iPad USB charger (rated for 2 amps at 5V - more than the device needed). However the device itself never became 100% charged (light turns green). In fact, the charge state went from yellow to red indicating the battery was being damaged by the charging process.
