About the author

Dr. Richard Horowitz is a board-certified internist and the medical director of the Hudson Valley Healing Arts Center, an integrative medical center which combines both classical and complementary approaches in the treatment of Lyme disease and other tick-borne disorders. He has treated over 13,000 Lyme and tick-borne disease patients in the last 30 years, with patients coming from all over the US, Canada, and Europe to his clinic. He is former Assistant Director of Medicine of Vassar Brothers Hospital in Poughkeepsie, N.Y., and is one of the founding members and past president elect of ILADS, the International Lyme and Associated Diseases Society. He is also past president of the ILADEF, the International Lyme and Associated Diseases Educational Foundation, a non-profit organization dedicated to the education of health care professionals on tick-borne diseases. Dr Horowitz has trained over 200 healthcare providers in diagnosing and caring for patients with treatment-resistant tick-borne disorders and was previously awarded the Humanitarian of the Year award by the Turn the Corner Foundation as well as awards from Project Lyme for his treatment of Lyme Disease. He has dedicated his life to helping those stricken with this devastating illness.
He is also the author of two best-selling books on Lyme disease, Why Can’t I Get Better? Solving the Mystery of Lyme and Chronic Disease (2013, St Martin's Press, NY Times Best Seller), and How Can I Get Better? An Action Plan for Treating Resistant Lyme and Chronic Disease (2017, St Martin's Press, National bestseller). These books incorporate recent scientific advances and explain in detail how healthcare providers can effectively diagnose and treat resistant chronic illness. In addition, he has co-authored peer-reviewed Lyme guidelines, and published in the medical literature on novel “persister” drugs for the treatment of Lyme disease and coinfections, as well as the role of the 16-point MSIDS model in chronic illness.
Other projects include Dr Horowitz serving as a consultant to governmental agencies around the world, including those in China, Australia, Belgium, France, the United Kingdom, and the United States. Dr Horowitz was a member of the HHS Tick-borne Disease Working Group and Co-chair of the “Other Tick-borne Diseases and Coinfections” subcommittee from 2017-2019 which provided recommendations to Congress to help the US improve the care of those suffering with tick-borne disorders. He recently served as a member of the HHS Babesia and Tick-borne Pathogen Subcommittee and was elected by Governor Cuomo to serve as a member of the NYS DOH Tick-borne disease Working Group in 2021.
Finally, Dr Horowitz recently published several scientific articles on classical and integrative solutions for COVID-19 which have been effective in helping those with acute and chronic illness, and he will be releasing his first novel, Starseed R/evolution, ‘The Awakening’ a cli-fi (science fiction/climate change) novel in the Fall of 2021, which will provide novel, potential solutions to our present climate crisis.

I would recommend that future editions of the ebook have direct links embedded to video presentations by Horowitz and other key Lyme researchers, practitioners, advocates, and patient experiences. For just one example, the series of videos by legendary pathologist Dr. Alan McDonald on borrelia and similar slow-growing bacteria in chronic infections are deeply illustrative of the survival mechanisms of these bacteria, and informative about the advances in detecting them--and they demonstrate the constant innovation and ongoing progress being made in the science. I found McDonald's perspective helpful in making doctors better understand what they are up against and for patients to visualize the same during their treatment. I would also be bolder and include links to the statements and views of more traditional IDSA thought leaders for comparison. I personally found that taking the time to listen to those viewpoints directly helped me to better grasp the flaws in rationale pointed out by ILADS members and supporters, and captured the selectivity in using evidence that informs and flaws the IDSA perspective.
Finally there are some good review articles showing up in the peer reviewed literature that should be cited as a follow up reading resource for those wanting more detail on the science, and again some of these articles have helpful takeaways for practitioners and patients. One such review is Keith Berndtson's 2013 article in International Journal of General Medicine entitled "Review of Evidence for Immune Invasion and Persistent Infection in Lyme Disease." Offering an appendix of such materials for readers that gets updated with new editions will be useful; the current section on recommended reading is much too brief and should be enhanced and annotated. Another appendix that would be useful would include more in depth tables addressing variations on the medication combos provided throughout the book (the current Tables are appreciated, but would be helpful to provide more detail on many more viable permutations, along with common dosage levels). Also more detailed advice and lessons learned about how and when to ramp up, when to lower what medications safely for what periods of time as needed while maintaining some therapeutic effectiveness would be helpful. Detail on what is effective as pulse therapy over what time frames for drugs like Tindamax and Flagyl would be well-received as there is much variability in the field on that, ranging from warnings against to regular pulses. That is all difficult territory for doctors and patients to knowledgeably navigate as Herxheimer, die-off reactions occur. Perhaps a more general section or appendix discussing polypharmacy with antibiotics and using antibiotics with slow growing organisms with chronic illness would be an asset, helping to increase practitioner understanding and confidence as that represents a different set of practices and principles than what is taught and that is routine for acute infection. It has become apparent that it is the multi-antibiotic approach along with other forms of system support from conventional and alternative medicine that will be needed for Lyme recovery, yet that is a boundary that I think is difficult for primary care physicians to cross, and forces patients to search for difficult to access specialty care.
