As a digital publisher (4+ years) we dabble in paper (about 450+ of our over 650 titles are in paper), but we do not try to push our books into bookstores. After having read In Cold Type, I'm very happy to understand the details better - other than a gut feeling.
What looks (from an outside person) as a horrible business strategy seems to be the same on the inside.
When one understands the size of the pie for paperbacks, and the costs associated with the process (before POD), one realizes why everyone is seeking more...
And there just isn't more to give.
In my opinion, one can't fault any of the players specifically. But, with understanding a person can provide new ideas to get around the problems. With digital efforts and sales, I believe publishers will have an opportunity to change the paradigm going into the future.
We shall see if it happens, or not.
